Neonatal Pneumomediastinum, Spinnaker-Sail Sign: A Case Report and Review of the Literature
Authors
Abstract:
Background: Pneumomediastinum (PM) occurring in approximately 0.1% of neonates is an asymptomatic accumulation of free air in the mediastinum. In this paper, we report on a preterm newborn experiencing a significant spontaneous pneumomediastinum subsequent to an uneventful vaginal delivery in the absence of resuscitation maneuvers. Case presentation:A-2560 g-male newborn was delivered vaginally to a 30-year-old mother at 34 weeks’ gestation following an uneventful twin pregnancy. Physical examination was unremarkable except for the presence of respiratory symptoms including dyspnea and oxygen desaturation (O2Sat<85%) which required nasal continuous positive airway pressure (nCPAP). Over the ensuing day, RDS was suspected and considering the possibility of secondary surfactant insufficiency, surfactant therapy was performed. A chest X-ray obtained after 24 hours was compatible with a pneumomediastinum without subcutaneous emphysema. By 96 hours of age, his respiratory distress began to resolve. Chest x-ray films obtained at the 5th day of birth showed complete reabsorption of the pneumomediastinum. Arterial blood gas measurements showed an acceptable level of gas exchange. Blood cultures taken at the referral hospital were sterile after 48 hours of incubation. He was eventually discharged at good condition after 13 days. This paper outlines the fundamentals of diagnosis and management of a pneumomediastinum in the neonate. Conclusion:In conclusion, spontaneous pneumomediastinum is a rare, benign condition which is usually over-diagnosed and over-treated. Conservative therapy with respiratory support and careful observation in the NICU are recommended until spontaneous resolution of the condition.

DOI: 10.1590/S1679-45082015AI3133 Pneumomediastinum in full-term newborn is associated with meconium aspiration syndrome, hyaline membrane disease, mechanical ventilation or traumas related with labor. The spontaneous neonatal pneunomediastinum is rare. We present imaging exams crucial for the diagnosis of a clinical case of pneumomediastinum in a newborn.(1,2) This is a radiology exam of a new...

Air-leak syndromes include pneumomediastinum, pneumothorax, pneumopericardium, pneumoperitoneum, pulmonary interstitial emphysema and subcutaneous emphysema. Pneumothorax and pneumomediastinum occur in 1 to 2% of normal neonates and they usually has no symptoms and resolves spontaneously. Here we report sudden fatal pneumomediastinum accompanied with pneumopericardium and pneumoperitoneum in no...
full textMALAKOPLAKIA OF THE COLON: A CASE REPORT AND LITERATURE REVIEW
Malakoplakia is a rare disease with undefined etiology that has been almost exclusively reported in urology and pathology journals. We are presenting a case of colonic malakoplakia in an I8-year old young man who was referred to us with a four-year history of intermittent rectorrhagia. The pathological examination of the endoscopically resected specimen revealed pathognomonic features of ma...

Malignant histiocytosis (MH) is a rare hematologic malignancy, especially in the first decade of life. The disease is clinically characterized by fever, hepatosplenomegaly, lymphadenopathy, pancytopenia and jaundice, and histologically by systemic proliferation of malignant histiocytes and hemophagocytosis. The prognosis is poor and often the diagnosis is not made before death. Because of ...

Spontaneous pneumomediastinum, defined as the presence of free air in the mediastinal structures, is a rare disease that usually take a good course with conservative therapy. However, clinicians are usually unfamiliar with the disease because of its infrequency. We report herein a case of spontaneous pneumomediastinum following an episode of violent cough. The patient was treated conservatively...
